Cosán Group’s mission is to deliver excellence in preventative care services for at-risk older adults to support successful aging in place. Cosán’s Care Management proves to enhance overall health outcomes for patients.
As a member of our Cosán Group team, you will be responsible for using our market-leading software platform to outreach to eligible patient to introduce valuable care management services, such as Chronic Care Management (CCM), Behavioral Health Integration (BHI), and Remote Physiological Monitoring (RPM) for qualifying patients as an extension of the Medical Providers you are assigned.

An extension of the provider to help educate our office's patients over the phone about insurance sponsored program options to better help manage their chronic conditions. As part of our amazing team, you will help your patient group by educating them on supplementary services such as Chronic Care Management (CCM), Behavioral Health Integration (BHI), Principal Care Management (PCM) and Remote Physiological Monitoring (RPM). Furthermore, you will empower patients to meet their goals, guide them in their healthcare journey and advocate for their needs by utilizing active listening skills, investigating their concerns, and communicating professionally with them about their health care options.

What’s your superpower? You will use your customer service experience, and communication skills to engage patients, educate them on services they qualify for, obtain consent for services according to Medicare requirements, and document consent compliantly in the patient’s record.
What does my day look like? You will work in a comfortable office-based setting introducing healthcare support services for elderly patients. There is no experience needed, training will be provided! Excellent customer service and basic ability to effectively navigate technology required.
What does the training look like? This is a knowledge-based opportunity, not a directly technical role other than developing specific expertise with our care management software application.
Required Qualifications
- Genuine interest in helping people
- At least 2 years' work experience in your existing role
- Have a knowledge of healthcare terms, conditions, roles, and basic care principles
- Intermediate to advanced computer skills as evidenced by ability to navigate multiple systems utilizing the same computer monitor, and quickly learn and navigate software programs and applications - including experience in Outlook and MS Office
- Must be highly motivated, results-oriented with strong skills in communicating, organizing, multi-tasking, and time management
- Effective communication, interpersonal skills, and problem solving
- Must possess telephonic and virtual communication skills, with an ability to verbally communicate clearly and effectively over the phone, primarily with elderly adults in a confident and compassion manner
- Ability to perform with a team to achieve daily goals and complete tasks to ensure that daily production goals and quality metrics are met within standards set forth
- Candidate must be able to pass a background check, have a positive attitude, adapt positively to change, be a team player, and be willing to learn new skills on a continuous basis.
- Must be able to commute to the office 5 days per week - office is located on Pistol Range Rd, Tampa, FL 33635
Preferred Qualifications:
- Bilingual in Spanish and English, however resumes must be submitted in English
- Preferred call center experience a plus
This role's responsibilities are:
- Initiate contact with patients who have been identified as eligible for care management services
- Provide patient education on care management support and services
- Document all patient communications (verbal or written) accurately, including all communications with patients and/or their caregivers
- Adhere to documentation protocols and best practices for daily workflow
- Exhibit excellent customer service skills demonstrated by positive feedback from customers and patients
- Demonstrate an ability to develop and build trusting relationships with patients ,have a true passion for caring for patients and guiding them in their health journeys
- Have an ability to stay flexible and quickly adapt to new processes, software changes, and priorities with a positive attitude and a willingness to learn new skills on a continuous basis
- Must be an empathetic and active listener with the ability to thrive both as a team member and independently in a fast-paced environment
- Inform your manager of all issues pertinent to the enrollment process and any known or perceived issues
- Ability to utilize multiple scripts for different clients and services
- Ability to follow processes and protocols
- Meet or exceed daily call expectations as directed by management
- Become skilled at using technology including the Cosan e-consent platform
- Excellent customer service skills demonstrated by positive feedback from customers and patients
- Contribute as a positive member of the department by supporting all members of the team in a productive and constructive manner
- Other duties as assigned
This role's physical/environmental demands are:
- While performing the duties of this job, the employee is required to sit for long periods of time in an office-based setting and occasionally required to lift up to 25 lbs
- Travel is typically not required for this position
Success in this role means:
- A minimum average of 12 calls attempts/hour
- A minimum of 2 average enrollments/hour
- Minimum Quality Assurance (QA) metrics must be maintained related to compliant use of scripting and documentation

Salary Range: Starting $18/hour with bonuses available. based off performance.
