The MSW Therapist will provide therapy, care management, and wrap-around clinical services to a group of high-risk, system-impacted young people ages 13-25 enrolled in our transformative programs at our New Earth Campus. This dynamic, resourceful, and detail-oriented individual will become a catalyst for our young people’s success and achievements and become a steadfast advocate for their progress. The Care Manager/ Therapist will be the client’s partner on his/her journey through the New Earth programming and will continue to provide aftercare and support to program alumni. The Care Manager/ Therapist will conduct assessments, maintain regular communication with New Earth members, work directly with New Earth staff and ensure wraparound, clinical services are being provided to students. As a New Earth team member, the Care Manager / Therapist upholds the New Earth mission, prioritizing high-quality and meaningful service to youth ages 13-25 who are impacted by the juvenile justice system, foster care system or are cross-system impacted. As a member of this team, the Therapist contributes to client intake and assessment, data tracking, resource delivery, and ongoing conversations about service fidelity and quality.
Duties & Responsibilities
- Assess client needs and eligibility for services. • Provide selected agency services to an identified caseload, referring clients to other services as appropriate. • Provide psychotherapy/counseling services to a diverse clinical population utilizing a variety of treatment modalities, including individual, group and family therapy. • Conducts intake assessment with new clients, including processing and filing intake questionnaires and narratives • Maintains current and acceptable standards of charting, meeting the requirements of State audits. • Takes concise and efficient progress notes and ensure daily record keeping • Maintains regular communication with active clients and alumni, including outreach to non-active members to check-in • Maintains a care management case load and a therapy caseload • Develops individualized service plans (ISP) to assist clients with goal setting • Collaborates with school partner to provide records of DIS counseling as requested through IEP.Works directly with center staff and educational staff to create an action plan that is “wrap-around” in nature ensuring the client’s success toward action plan goals. • Completes required reports and updates appropriate staff on a weekly, bi-weekly, and/or monthly basis, as needed/requested • Adaptable and flexible to meet participants and/or families in a variety of settings including but not limited to schools, worksites, home, court, other community-based organizations, or via digital communication • Introduces intervention services when necessary, including but not limited to restorative justice • Develops relationships with New Earth students, including an understanding of their outside support, to identify program completion hurdles and resource needs • Engages in weekly or bi-weekly case conferencing with internal New Earth team to discuss New Earth member’s needs to ensure most effective service delivery possible • Develops and maintains relationships with outside wraparound support services to ensure continuity of care and collaboration • Assists in the planning and implementation of a life skills curriculum • Assists students in gaining career development and college readiness skills • Assists in the implementation of a leadership curriculum • Implements Nonviolent Crisis Prevention and Intervention (CPI) strategies to prevent and deescalate harmful behavior, reducing the risk of injury and ensuring the safety of client or others • Circulates throughout classroom spaces and general areas to serve as a consistent and supportive presence to students throughout the school day • Provides mediation in conflict situations between students and other parties to prevent behavioral crisis • Collaborates with Special Education Team, Instructional Team, & School Admin to assist in the development, implementation, and modification of behavioral intervention and support plans • Provides weekly status reports to leadership team • Regularly complete download and storage protocols for all client data. Participates in developing agency policies and procedures with regards to social work practices and program needs. • Transport clients as needed to ensure continuity of care • Participate in individual supervision/consultation as needed. Screen and assign cases in • supervisor's absence as needed. • Fulfill productivity requirements on a weekly basis • Submit documentation to support productivity requirements in a timely manner • Performs all other duties assigned

These responsibilities include recognizing signs of maltreatment, abuse, and neglect. • Understand approaches of suicide/homicide risk assessment, including collecting accurate assessment information, formulating risk, developing a treatment and services plan, managing care (i.e., immediate response to identified risk level), and understanding legal and regulatory issues related to suicidality/homicidality. • Understand the basic rights of client's served including the California Client's Bill of Rights for Mental Health. • Understand how to work collaboratively with families to meet the needs of children and youth through family-based interventions • Understand diversity within and across cultures and provide an inclusive, welcoming, and respectful environment where all children, youth, and families can take pride in their cultural identities, beliefs, and practices. • Apply social work ethical principles to guide professional practice. Social workers have an obligation to conduct themselves ethically and to engage in ethical decision making. Education • BA or MA in Family Therapy, Psychology or Social Work • MSW, MFT, PPSC, or school counseling credentials required (MSW preferred) • At least four (4) years working with at-promise or system-involved youth License/Certification • MSW, MFT, PPSC, or school counseling credentials required (MSW preferred)
